Immunization assessment report (CDPH)

Last updated: November 3, 2026

Each fall, California schools report immunization counts for TK/kindergarten and 7th grade students to the California Department of Public Health (CDPH) in CAIR Hub. Scout fills in the numbers for both forms, the TK/Kindergarten Summary Sheet (IMM-236) and the 7th Grade Summary Report, so you can copy them into CAIR Hub.

Opening the report

  1. Click Reports in the sidebar.

  2. Click California government reporting.

  3. Click Immunization assessment (CDPH) in the left sidebar.

  4. Pick the school year.

  5. If your district has more than one school, click the school's tab.

There's no download. Copy each number into CAIR Hub by hand.

Which students are counted

The line next to the school year dropdown shows the date Scout counts students on.

  • For the current school year, the date is today.

  • For a past school year, it's the last day of that year. For a future school year, it's the first day.

  • A student counts if they were enrolled on that date in TK, kindergarten or 7th grade.

  • No-shows aren't counted.

  • A student enrolled at two schools on that date counts at the school they enrolled at most recently.

  • Scout uses the doses and overrides on file today, even for a past school year.

Which row each student goes on

Scout places each student on a row from the status on their Compliance Status tab.

Row on the form

Students on it

All Required Vaccine Doses

Students with every required dose. On the 7th grade form, students with a Tdap dose.

Permanent Medical Exemption

Students with the MED-P override

IEP Services

Students with the IEP override

Independent Study

Students with the IND override, and students at a school with no classroom-based instruction

Home-based Private School

Students with the HOME override

Conditional – Missing Doses Not Currently Due

Students with the COND override. TK/K form only.

Temporary Medical Exemption

Students with the MED-T override

Overdue – Needs Doses Now

Students missing required doses. On the 7th grade form, students missing Tdap.

  • The 7th grade form only checks Tdap. A 7th grader with a Tdap dose goes on All Required Vaccine Doses, even if they're missing other doses.

  • Scout only puts a student on the Conditional row when staff set the COND override. A TK/K student who is missing doses that aren't due yet goes on Overdue – Needs Doses Now until you set COND on their Compliance Status tab.

  • Scout trusts overrides. A student with an override goes on that override's row, whatever doses are on file.

Checking which students are on a row

Click any number on a form to see the students behind it. Click a student's name to open their Immunizations page in a new tab.

The Notes column shows the vaccines each student is missing, or why they aren't on the form.

Missing doses by vaccine (TK/K form)

The Missing Doses By Vaccine box counts how many students are missing each vaccine: Polio, DTaP, MMR, Hepatitis B and Varicella. It includes every student placed below All Required Vaccine Doses, and it uses the doses on file, even for students with an override.

Students who aren't on the form

The Total on each form counts only the students placed on a row. When some students don't fit any row, an amber note under the Total says how many. Click the note to see those students and why each one isn't on the form. Fix them in Scout, or count them by hand in CAIR Hub.

Note

Cause

Fix

Medical exemption override doesn't say whether it's permanent or temporary

The student has the older MED override.

On the student's Compliance Status tab, click Clear Override, then pick MED-P or MED-T.

The 7th grade form has no conditional row

The 7th grader has the COND override.

Clear the override if the student no longer needs it, or count them by hand.

Missing date of birth

Scout can't check the student without a date of birth.

Add the date of birth on the student's profile.

Missing date of birth, so missing doses can't be counted

The student has an override, but Scout needs the date of birth to count their missing doses.

Add the date of birth on the student's profile.

Personal beliefs exemptions from before 2016 don't carry into TK/K or 7th grade

The student has a personal beliefs exemption, which California no longer accepts at these grades.

Count the student by hand.
